What You Will Do:
The Clinical Informatics Pharmacist (CIP) is an expert in human factors, patient safety, and the use of technology to optimize care delivery processes and effectively communicate patient care activity. The CIP specializes in the medication process, from ordering to administration, and is responsible for effective process design, development, and implementation of the Electronic Medical Record and Clinical Decision Support. As well, this role supports pharmacists, physicians, nurses and other clinicians as the resource for requests related to informatics. The CIP demonstrates independent decision making as appropriate in the evaluation of requests for system improvements and determines feasibility and advisability of system changes. The CIP works closely with clinicians, departmental leadership, quality, patient safety, accreditation and licensure departments and Information Services staff to clarify needs, prioritize requests, design workflow to fit with electronic systems, and execute change and facilitate adaptation to the Electronic Medical Record.
Assists in order set management:
Development and maintenance of overall principles for safe design and use of order sets, with a focus on medications
Is a resource to the clinical pharmacist in the development and maintenance of standard order set clinical content
Identified leader as resource for requests for changes, additional orders and order sets
Guides new providers and clinical users through the system/process
Responsible as thought leader for maximizing technology design to achieve the best clinical outcomes. Leads the organizational design, implementation and evaluation of Clinical Decision Support
Content expert on the literature related to the use of the Electronic Medical Record and Clinical Decision Support to meet organizational quality and patient safety goals
Participates in development, monitoring and evaluation of decision support for continuous improvement, particularly for medications
Partners with clinicians to identify and facilitate advanced clinical decision support implementation within the electronic medical record in order to meet clinical goals
Participates in quality projects and on assigned committees to provide expertise on using the clinical information system to achieve goals
Collaborates with regulatory authorities on education for safe and effective use of the EMR and other informatics systems
Participates in functional testing for human factor issues before and after system upgrades and improvements
Problem solves application issues and works through barriers to achieve results by collaborating effectively with users and the information technology staff
Communicates effectively with all members of the healthcare delivery team and senior leadership
Accountable for the successful utilization of order sets and clinical decision support to drive best practices
Effectively communicate thoughts, feelings and/or information to a specific group as well as knowledgeable use of presentation devices
Demonstrates proficiency in conducting, coordinating and publishing research in healthcare informatics
PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS
A pharmacist with outstanding interpersonal and presentation skills, poise, and maturity, can work with all levels of employees, physicians and executives, and is able to engage colleagues on challenging and potentially difficult topics related to the use of the electronic medical record in clinical practice. Must be comfortable working with adult patient populations and age ranges from birth to geriatric, including patients with communication barriers and/or sensory impairments, have the ability to identify appropriate measures and make necessary accommodations to meet the needs of the patient and their age category. Also, one must evaluate and address patient population and patient-specific variables, assessing if the technical/human interface is appropriate for achieving the desired therapeutic outcome. Apply the patient specific variables to patient safety activities. Note: special consideration should be given to the requirements of the neonatal, pediatric and geriatric patient populations.In addition to Clinical Informatics Activities, the CIP must also be familiar with clinical/staff pharmacist duties (see clinical/staff pharmacist job description). Some of the required duties include, but are not limited to preparing and dispensing drug orders per physician request according to established policies, procedures, and protocols:
Interprets drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles.
Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately.
Issues controlled substances to patient care areas and maintains records as required by law.
Ensures safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols.
Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
Assists with pharmacokinetics consult service and drug dosing per hospital protocol.
Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as needed.
Reads, extracts and interprets information in patient charts accurately.
Detects and reports suspected adverse drug reactions accurately and in a timely manner.
Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
Provides accurate, adequate, and timely drug information to other members of the hospital’s professional staff.
Provides drug education to patients and their families.
Documents all clinical activities and interventions accurately and completely.
Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.
Participates in the development and presentation of orientation, education, and training programs for the medical staff, nursing service, and other hospital staff.
Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.
Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
Completes all assigned medication storage area inspections in a timely manner. Identifies and replaces outdated and unusable drugs.
Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window.
Keeps pharmacy areas and equipment clean, neat, and well-organized.
Oversees operation of the pharmacy in the absence of the Clinical Pharmacy Manager as necessary.
Maintains competency required for current job title/position.
Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files.
Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.
Completes all competency/skills assessment requirements.
Develops and implements initiatives to ensure safe and cost-effective management of pharmaceuticals in collaboration with the health care team.
Develops, implements, and actively participates in clinical research projects and other scholarly activities as appropriate.
Responsible for special projects pertaining to specific areas of interest and/or expertise; for conducting in-depth and highly specific research reviews of drug‑related literature and ability to make presentations on rational drug therapy.
Responsible for some inventory control/purchasing activity as required to maintain departmental inventory at appropriate levels.
Actively participates in the accomplishment and ensures the implementation of the pharmacy service’s scope of practice.
What You Will Need:
Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree or Pharm.D. Current licensed to practice pharmacy in the state of Ohio.
Hospital/clinical informatics experience preferred.
Demonstrates willingness, ability and availability to attend training as requested by supervisor or manager.
Demonstrates ability to implement learned skills and abilities through "on the job" efforts and/or professional certifications.
Achieves professional certifications as requested by manager or supervisor in accordance with all Fireland's training policies and procedures.
Ability to create effective procedure manuals and business correspondence.
Ability to use time and project management skills to complete projects and tasks on time and within budget. Maintains accurate documentation of projects through use of department approved forms.
Ability to work independently with minimal direct supervision. Ability to work cooperatively with pharmacy staff, hospital staff, and physicians.
Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
Ability to set priorities, make critical decisions and respond quickly to emergency requests.
Fosters a team environment by providing orientation and training to new team members.

Punctual attendance at assigned work location is required.
Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of Medical Center emergency codes.
Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
