SOCIAL WORK, LCSW
POSITION SUMMARY
Reports To:
Practice Manager
Summary:
To promote and provide optimal quality patient care and ensure the efficient and cost effective movement of patients through the health care continuum while promoting patient, family and staff satisfaction. This role integrates and coordinates with utilization management, care facilitation and discharge planning functions of patients in stages of preconception, perinatal, and family fostering. Social Work shall provide services to patients who have social, psychological, environmental, and/or economic needs related to the impact of admission, diagnosis, treatment and/or post hospitalization. Services provided are, but not limited to: psychosocial assessment, discharge planning, short term counseling, consultation, crisis intervention, education, community liaison, information and referral, coordination of services, and advocacy.
Work Schedule:
Must be able to occasionally work more than 40 hours per week.
SKILLS & ABILITIES
Education
Master's Degree in Social Work.
Experience
One to two years related experience preferred.
Computer Skills
Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility.
Must exhibit basic computer knowledge. Knowledge in using electronic medical records and other databases preferred.
Certificates & Licenses
Must be a Social Worker licensed by the State of Indiana, or eligible to take exam and pass within 6 months of hire.
Other Requirements
Should take no longer than 3 months to become proficient in this position. Knowledge of changes in regulations and requirements. Knowledge of healthcare delivery system, medical terminology, screening material, assessment skills, patient safety and quality initiative, management and treatment protocols and regulatory guidelines. Reading and math skills to analyze data, calculate results, and prepare graphs. This position requires frequent standing, walking, sitting, lifting and carrying 0-10 lbs and pushing and pulling 0-12 lbs.
