HOME HEALTH INTAKE COORDINATOR

The Home Health Intake Coordinator serves as the primary point of contact for patient referrals, ensuring a seamless and efficient intake process. This role is responsible for evaluating patient needs, verifying eligibility, obtaining necessary documentation, and coordinating with medical staff, patients, and caregivers to initiate care services. The ideal candidate has excellent communication, organizational skills, and knowledge of home health regulations and practices.
