with openness and wiliness to investigate when a problem is identified or arises with an obligation to act. Nurses are to help and guide consumer, guardians, and staff at any time to facilitate health and welfare services.

The LVN Nurse is to oversee the nursing department ensuring quality of care and consistency of services provided by all nursing staff. Ensure all Company, state and Federal guidelines are met, documentation, charts, reporting and billing is done efficiently and accurately.

Essential Duties and Responsibilities:

1. Observe, report, and treat appropriately to the physical and behavioral needs of the consumers, providing first-aid and other medical treatment as indicated.

a. Conduct body checks and completes physical review of each Consumer’s health status.

b. Attend Medical appointments when necessary.

2. Maintain direct communication with the attending physician by reporting physical changes and physical condition of consumers.

3. Assure that physician orders are transcribed correctly, and that accurate documentation is maintained.

4. Report and maintain communication with the ISP Team regarding physical conditions of consumers, and of physician’s findings and recommendations.

5. Maintain current record of each consumer’s scheduled medical appointments and assures that follow-up and routine evaluations’ are completed.

6. Participate as a member of the ISP in the formulation of consumer’s individualized service plan.

7. Provide training to consumer’s families and ISP members in accordance with individual’s health related issues.

8. Provide training to newly hired staff at orientation training and ongoing training to all staff, in the areas of self-medication, medication safety and individualized needs.

9. Maintain familiarity with all Standards of Principles.

10. Conduct complete focus assessments, including self-medication on each consumer upon admission, and quarterly thereafter, (or as deemed necessary by the ISP). Completes health care plans based on consumer’s health care needs as determined by assessment information.

11. Provide training to direct services provider of family members who are responsible for performing, motoring, reporting, and documenting specific individual treatment plans for a specific individual treatment plans for a specific consumer.

12. Conduct review of each consumer’s record at least monthly, including updating of Service Strategies.

13. Observe medication supervision in the homes weekly to assure that accuracy, technique and training is occurring without error.

14. Conduct weekly checks of medication storage, med counts, and medication administration records to assure correct compliance within the Standards.

15. Complete special needs forms for all consumers and keep updated as changes occur.

16. Complete all assessments and will have to the IPC/FC Coordinators seven days before the PDP and 3 days before the quarterly.

17. Assist in the maintaining, responsibility, and accountability for the proper charting and documentations of care and of medications and treatments.

a. Establish new consumer charts as necessary

18. Complete required forms and documents in accordance with company policy and state and/or federal regulations.

19. Complete and submit billing per required percentage on a weekly basis.

Additional Requirements:

LVN’s nurses will work under the guidelines of the Work Agreement. They are to work a 38 to 40 hour work week. Is available 24/7 to respond to nursing needs as determined by the RN.

They must attend all in-service and continuing education/training required or indicated to meet the needs of the persons enrolled in the program and the requirement of the HCS principals. Attend schedule staff meeting unless otherwise authorized.

At no time will falsification of any document be tolerated. Falsification is considered Medicaid fraud. Termination will occur and charges will be filed with the authorities.

Your duties will require you to be alert at all times. It is important for you to be able to mentally access yourself, your environment and the status of others.

Minimum Qualification: Licensed Nurse in the State of Texas, Valid Texas Drivers License and current automobile insurance. Excellent computer skills required. Good communication and organizational skills. Be energetic with a positive can-do attitude. Exhibit a professional demeanor.

Physical Demands: May be required to work non-standard hours. Employee is frequently require to stand, walk, use hands or fingers, reach with hands and arms, feel see, talk and hear. Employee is occasionally exposed to other body fluids, fumes, or airborne particles and toxic or caustic chemicals.
