RN Nurse Manager Assisted Living
RN Nurse Manager must have experience in handling administrative functions and organizing processes to maintain compliance with regulations.

- Willingness to focus upon hire on gaining full knowledge of the NYS DOH Regulations for an Assisted Living Residence with Certifications for both Special Needs Assisted Living and Enhanced Assisted Living
- Prior experience managing staff at either a nursing home or assisted living residence
- The NM oversees, implements, and uses his/her nursing knowledge to promote and maintain the coordination of care and services to ensure that Residents’ needs are adequately and appropriately met.
- Collaborating closely with the Leadership Team, the NM plays a critical part in the admission and retention of Residents. To remain in regulatory compliance with NYS DOH Regulations, the NM holds an integral role in adhering to all requirements for our license.
- Must implement and maintain Quality Assurance Program.
RN Nurse Manager is responsible for leading the Wellness Team providing support and
assistance to Nurses and Med Assistants. Leading by example, the NM exhibits the highest
level of professionalism and customer service in resolving problems while promoting
ongoing communication and responses to all resident/family questions and requests.
The NM is responsible for daily communication with the Executive Director sharing pertinent information.
Qualifications:
Graduate from an accredited school of nursing
Current New York State Registered Nursing License in good standing
Meets all NYS Department of Health requirements.
A minimum of 3+ years’ experience preferably in an assisted living community or skilled nursing facility caring for a geriatric population with cognitive impairment.
Excellent verbal and written communication skills with ability to utilize all Microsoft Office Programs.
Ability to accept responsibility for documentation, creation/editing of forms/policies and QA program to create a smooth operation in Wellness Office.
Delegate tasks to each shift to meet all DOH Regulations.
Represent the facility in a polished and professional manner, always maintaining conservative business attire.
Responsibilities and Duties
Responsible for submission and monitoring of all DOH required duties and to maintain communication with the Executive Director on status of same on an ongoing basis.
Lead Quality Assurance Program and Committee. Review, maintain and document relevant records as per DOH policy and procedure.
Maintain confidentiality of all pertinent Resident information.
Demonstrate an understanding of Residents’ preferences and honors Resident Rights.
Along with the Executive Director, responsible for the NYS DOH Wellness Plan of Correction following survey, rectifying areas with deficiencies and implementing QA as needed.
Delegate or visit any Resident admitted to Hospital or SNF for Rehabilitation.
Complete all service plans and assessments.
Implement and monitor daily Shift Change Reports/Progress Notes to maintain open communication with Staff.
Become familiar with all paperwork - both 80th Street and DOH mandated forms.
Become familiar with each Resident’s chart and all paperwork both upon admission and on an ongoing basis to be best able to communicate with family, physicians and other providers in a professional manner ensuring nurses’ notes are up to date.
Provide orientation, supervision and ongoing coaching of Nurses, Med Assistants and Care Managers.
Coordinate work schedules and staffing to avoid overtime.
Inform the Executive Director/Assistant Executive Director and Resident Manager of any care level changes and incidents.
Observe Residents for changes in physical, social or mental status, and report such variables to physicians and family as required.
Lead appropriate in-service programs for staff tailoring each to learning needs.
Responsible for submission of:
- Equipment Waivers for Residents to the NYS DOH
- Incident Reporting as per DOH guidelines and our policy.
Update tracking tools and records on Shared Drive. Prepares and maintains resident’s files and reports.
Adheres to all established facility policies and procedures and other duties as assigned.
Adheres to New York State Department of Health regulatory standards.
Attend all appropriate training, meetings and team initiatives as required.
Coordinates services with other healthcare facilities to ensure the continuum of care for our residents who may have temporarily transferred to other facilities.
Maintain resident confidentiality as per HIPPA guidelines.
Assist with any mandated testing and/or reporting as required during a Health Emergency.
Facilitate Communication:
Collaborate with Leadership Team daily & communicate any medical issues to physician and family/resident representative as soon as possible.
Quality Assurance:
Responsible for Quality Assurance Program, including performing assigned audits.

Education Experience Licensure
Graduate of accredited Minimum 3+ years Current NYS RN & BLS
School of Nursing (RN) experience in geriatrics required
Along with history of handling
