Position SummaryWe are seeking a dedicated Social Worker to join our interdisciplinary team. The successful candidate will be responsible for providing comprehensive support to our residents and their families, ensuring high-quality care and services.Key Responsibilities:Coordinate services with various healthcare providers, including dental, optometry, and other necessary referrals.Provide professional counseling and emotional support to residents and their families.Investigate, intervene, and document resident behaviors in accordance with community protocols.Evaluate and address residents' psychosocial needs through thorough assessments.Manage discharge planning processes to ensure smooth transitions for residents.Assist residents in accessing financial benefits they are entitled to receive.Maintain accurate and up-to-date Social Services records for all residents.Perform additional duties as assigned by management.
