Job Description:Job SummaryThe Heart Failure Program Coordinator is responsible for the coordination and maintenance of a comprehensive Heart Failure program that complies with national standards for the deliverance of excellent quality of care for Heart Failure patients as dictated by the American College of Cardiology (ACC), and governmental regulation for Medicare Compliance. The Coordinator partners with the physician leaders of cardiology services and other related service line and clinical program leaders to ensure development of a comprehensive, market competitive, and leading edge clinical program at Baptist Health Louisville and at any other affiliated sites across the region as deemed appropriate if/when applicable.Key Responsibilities and Essential FunctionsThe following is intended to describe the general nature and level of work being performed by individuals assigned to this job by listing the major components for this position (i.e., key responsibilities and essential duties).
