We are seeking an experienced, compassionate RN or LPN to join a long-term home-care case in Riverhead NY. The patient is ventilator dependent and requires skilled nursing care in a warm, loving, dog friendly family home.

Location: Riverhead NY

Rate: $56.00per hour for LPN, $63.00 per hour for RN, medically fragile rate

Schedule: Multiple shifts open,

Home Environment: Warm, supportive, dog friendly family

Case Type: 24/7 ventilator dependent patient

IMPORTANT: Must be an active NY Medicaid provider and have an NPI number

Requirements:

Active NY RN or LPN license

Must be enrolled as a Medicaid provider

Valid NPI number required

Ventilator experience required

Proficient with suctioning, trach care, airway management, respiratory monitoring

Reliability and professionalism are essential

Home-care experience preferred

Patient Care Responsibilities:

Continuous ventilator monitoring

Suctioning and airway clearance

Trach care and respiratory assessments

Medication administration

Documentation and communication with family and care team

Safety oversight in a medically fragile environment
