SCHEDULE

Client has 48 Total hours per week

Monday to Friday: 09:00 AM to 05:00 PM

Saturday & Sunday 11:00 AM to 03:00 PM

SERVICES NEEDED

Assistance with all of his ADL and IADL including, but not limited to:

bathing, dressing, transitions, community errands necessary to remain living in the community, meal preparation, and transitions.

COMPENSATION:

The position is w2: $17/hr

CAREGIVER QUALIFICATIONS

1. Must be Female in Gender

2. The caregiver should have CNA

3. The caregiver should have at least 3 years of experience in Personal Assistance Services (PAS)

4. Experience with using Hoyer Lift (Added bonus, if not she will be trained)

5. We need a reliable, dependable person

6. The Caregiver should drive their own car

G. Copy of Your First Aid/CPR

H. Copy of all your other qualifications (CNA, CMT, HHA etc)

I. Copy of your Physical Test and PPD (Tuberculosis test)

J. You will be required to do another Background Check
