Unique, innovative human service agency based in Merrimac, MA serving adults with disabilities is seeking to fill the following position:

Part Time LPN

Description:

· Provide oversight, supports, and guidance to Shared Living Providers to ensure that the safety, health, dental, and wellness needs of all Individuals are met at all times

· Provide coordination, oversights, and documentation related to routine physical, dental, eye and ear exams, doctor, and follow-up appointments and emergency medical care

· Ensure that prescription and over the counter medication is dispensed as ordered, and maintain current physician’s orders and Medication Charts

· Provide training and consultation to Shared Living Providers and Staff on new skilled nursing functions, such as feeding tubes

· Promote a healthy lifestyle and preventative health care measures

· Assist with obtaining/ordering medical supplies and adaptive equipment

· Ensure compliance with all DDS and Agency policies, guidelines, and licensing requirements, and enter related medical data as required

· Provide First Aid/medical assistance as needed and appropriate

Supervision:

Works as part of a team under the supervision of the Executive Director

Education:

MA LPN license

Associate’s Degree in Nursing or related field

Experience:

Qualified applicants should have at least two years experience in providing supports to adults with developmental disabilities, physical challenges and/or mental health issues. DDNA certification strongly preferred.

Skills/Qualities:

· Extensive knowledge of nursing procedures, practices, and guidelines

· Solid knowledge of developmental disabilities and autism

· High degree of coachability, flexibility, organization, and attention to detail

· Effective written and oral communication

· Ability to maintain medical records and adhere by all applicable policies, guidelines, etc

· Ability to work independently and as part of a team and collaboratively with collaterals and medical providers

· Ability to maintain confidentiality, prioritize, and multi-task

Knowledge:

· Understanding of DDS policies, procedures, and regulations

· Current CPR, First Aid certification

· Working knowledge of nursing, medical, health, and wellness procedures, policies, and resources

· Knowledge of Insurance, MassHealth, and Medicare practices and requirements
