Position Overview
The Housing Specialist supports members in achieving both immediate and long-term housing stability. This role serves as a liaison between members, housing providers, landlords, brokers, and government agencies to facilitate housing placement and retention. The Housing Specialist acts as a subject matter expert for housing-related services and provides support to the care management team based on case complexity.
This is a hands-on, predominantly field-based role requiring travel throughout Manhattan. The position involves extensive outreach, advocacy, coordination, and documentation to ensure members access safe, stable, and appropriate housing resources.
Key Responsibilities
Conduct phone and in-person outreach to vulnerable members experiencing housing instability or homelessness
Perform comprehensive housing assessments to determine member needs and appropriate services
Assist members with housing applications, including NYCHA, HRA, and NY/NY programs
Gather, review, and organize required documentation and records
Prepare members for housing interviews and accompany them to appointments when needed
Serve as liaison with landlords, real estate brokers, housing providers, and government agencies
Coordinate with providers to obtain necessary clinical and psychosocial documentation
Advocate on behalf of members to address housing barriers and resolve challenges
Educate members and caregivers on tenant rights, responsibilities, and housing processes
Support care management teams by providing housing expertise and case consultation
Attend external stakeholder meetings and collaborate with community-based organizations
Assist in expanding and maintaining referral networks
Research and identify funding and housing opportunities for members
Maintain accurate, timely, and comprehensive case documentation
Comply with all organizational policies, training requirements, and performance evaluations
Perform other duties as assigned by management
Qualifications & Skills
Experience processing housing applications (NYCHA, HRA, NY/NY preferred)
Significant experience working with individuals affected by homelessness, mental health challenges, substance use, and socioeconomic barriers
Knowledge of tenant rights and responsibilities
Strong advocacy, problem-solving, and critical-thinking skills
Excellent written and verbal communication skills
Proficiency navigating multiple computer systems and web-based applications
Ability to work independently in a fast-paced, field-based environment
Bilingual English/Spanish preferred
High level of integrity, professionalism, and customer focus
