The program brings together health and psychosocial expertise to address the full range of factors that influence wellbeing, with a strong focus on equity, access, and person-centred care.Your role, your impactAs a Social Worker in the My Care Partners Program, you will support people navigating the psychosocial challenges that often sit alongside chronic and complex health conditions. Your work will help reduce barriers to care, strengthen individual resilience, and support people to feel safer, more connected, and more empowered in their daily lives.Working closely with Registered Nurse Care Coordinators and primary care teams, you will contribute to integrated, holistic care that responds to both health and social needs.You will:Deliver psychosocial support and care coordination for people with chronic and complex health conditions in the communityConduct holistic psychosocial and risk assessments to identify factors impacting health, safety and wellbeingDevelop and implement individualised care plans in partnership with clients, families and multidisciplinary teamsProvide short to medium-term therapeutic support, including counselling for anxiety, depression, grief, stress, family violence and adjustment-related concernsSupport goal setting and self-management, strengthening people’s capacity to navigate health and community servicesAssist with care navigation, including support with housing, income support, NDIS and other service systemsCoordinate access to supplementary services funding to reduce financial barriers and promote equitable access to careParticipate in case conferencing and multidisciplinary meetings to enhance continuity and quality of careMaintain accurate clinical documentation and deliver services in line with professional, ethical and legislative standardsContribute to quality improvement initiatives and positive team cultureWho we’re looking forYou are a qualified Social Worker who brings empathy, strong professional boundaries and a commitment to person-centred, trauma-informed practice. You are comfortable working with complexity and value collaboration as much as autonomy.You will bring:Experience supporting people with complex psychosocial needs, including mental health and/or substance useStrong assessment, counselling and care coordination skillsA trauma-informed, culturally responsive and inclusive approachConfidence working within multidisciplinary health teams and primary care settingsHighly developed communication, organisation and time‑management skillsSound judgement, emotional intelligence and the ability to work within scope of practiceMandatory requirementsBachelor’s degree in Social Work, providing eligibility for AASW membershipMinimum two years’ experience working as a qualified Social WorkerCurrent driver’s licence and willingness to travel for community and home-based workWhat will make you stand outExperience working in community or primary health settingsKnowledge of the social model of health and social determinants of healthExperience working with culturally and linguistically diverse communitiesConfidence supporting people to navigate complex service systemsWhat’s in it for youAt Each you’ll be supported to be your best.
