Assists YVFWC clients in obtaining the social and health services they need in an integrated and coordinated manner.What we are looking for in a Case Manager:Interest in supporting the development of expectant parents and families with young children 0-5 years;

An open and clear communicator who is able to build trusting relationships with clients, program and agency staff, and community partners;

Desire to work within a collaborative team environment;

Experience in case management, home-visiting, and/or early learning preferred;

Bachelor's degree and Bilingual in Spanish preferred;

What you will do:Work with the Parents as Teachers program, an evidence-based home visiting program model;

Provide case management to clients, which may include sharing resources and referrals, goal-setting, family well-being support;

Partner with clients to provide education around their child's development while encouraging positive parent-child interactions, conduct regular developmental screenings and assessments for children and families;

Provide proper file documentation and case notes, while protecting client's Personal Health Information;

Collaborate with fellow Parents as Teachers program staff to provide services to families in accordance to current CDC, state, and YVFWC guidelines around COVID-19

Qualifications:Education: Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology or Child Development. 4 years of related experience may substitute for Bachelor's Degree.

Experience: Two years of experience working in community social services, public health services, crisis intervention, outreach or linkage programs.

Licenses/Certificates/Registration: Current Washington driver's license and proof of automobile liability insurance coverage.

Knowledge/Skills/Abilities Required: Basic bilingual proficiency in verbal and written communication preferred. Must have demonstrated ability to communicate at level 10 on the ALTA language proficiency scale in order to receive bilingual differential. Effective verbal, written and listening communication skills. Strong customer relations skills including the ability to manage stressful situations. Knowledge of medical terminology required. Knowledge of cultural differences and impact on appropriate care provided to patients. Knowledge of local social and health services. Knowledge of how to access local community agencies and resources. Ability to develop effective working relationships with social service providers and partners. Ability to effectively interact with a variety of clients and employees in a professional manner. Basic proficiency with a variety of computer programs including, Microsoft Outlook, Word and Excel.
