Growing together.We are looking for a Clinical Administrative Coordinator as an addition to the Practice Extend Team. You will work alongside our Interdisciplinary Clinical Team to improve the health outcomes of the patients and providers we serve. The Practice Extend Clinical Administrative Coordinator is responsible for engaging high-risk patients for Comprehensive Medication Reviews with the Clinical Pharmacist, scheduling PCP appointments for patients on their behalf, assisting with administrative tasks, and communicating with provider offices regarding patient medical history and current needs.This position is full time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work three days a week from 08:00am – 05:00pm and two days from 10:00am – 07:00pm. It may be necessary, given the business need, to work occasional overtime, weekends and holidays.We offer 4 weeks of on-the-job training. The hours of training will be 8:00am – 5:00pm, Monday – Friday.If you are located within the state of AZ, CO or TX, you will have the flexibility to work remotely as you take on some tough challenges.Primary Responsibilities:

Work with a Clinical Care Team to support patient and provider engagement

Engage high-risk patients telephonically to schedule appointments with Clinical Care Team

Utilize multiple technology systems and outreach to provider offices and/or pharmacies as needed to capture pertinent information for patient engagement

Support patients by scheduling PCP appointments on their behalf

Prepare patient cases in workflow management platform for Care Team patient engagement

Facilitate exchange of clinical information between Care Team and provider offices to ensure identified patient concerns and needs are addressed by provider offices

Communicate with provider offices to problem-solve patient issues and/or facilitate communication between practice staff and Care Team

Notify Care Team of relevant information gathered during calls to patient that may affect the patient’s disease state or medication regimen

Develop strong relationships with provider offices participating with Practice Extend program by communicating patient needs

Build rapport and establish trusted relationships with patients via daily outbound and inbound calls

High School Diploma / GED

Must be 18 years of age OR older

2+ years of Prior Medical / Healthcare experience in a hospital, pharmacy, physician's office, or healthcare insurance.

2+ years of experience working with medical terminology

1+ years of experience managing high volume patient communication via phone.

1+ years of experience making high volume outbound calls to patients

Computer proficiency, to include strong data entry and utilizing Microsoft Outlook

Computer proficiency with direct messaging applications (Microsoft Teams, Webex, etc.)

Ability to type and navigate a Windows-based environment simultaneously

Experience working with and understanding HIPAA guidelines

Ability to travel up to 10% to local offices.

Ability to work Monday – Friday, three days a week from 08:00AM – 05:00PM and two days from 10:00AM – 07:00PM.

Preferred Qualifications:

Pharmacy Tech, Pharmacy Tech for Insurance Company, or Medical Assistant

Experience making outbound cold calls to patients

Managed Care Experience

Bilingual fluency in English and Spanish

Telecommuting Requirements:

Reside within AZ, CO, or TX

Ability to keep all company sensitive documents secure (if applicable)

Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Soft Skills:

Able to take initiative and work independently

Adaptable, flexible, and able to incorporate frequent process changes into established workflows within a fast-paced, dynamic environment

Strong communication skills with patients, providers, and other healthcare professionals

Colorado Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
