Monday through Friday; role is per dime so exact days requested to work may variesATTRIBUTESFive (5) years of acute hospital care experienceExcellent oral and written communication and interpersonal skillsStrong problem solving, decision making and critical thinking skillsAbility to recognize and manage emergenciesStrong knowledge of specific chronic care regimenTime management and organizational skillsMust be self-directed and able to collaborate RESPONSIBILITIESPatient Care. Serve as the patient's first line of communication throughout the entire treatment program; coordinate multiple and complex technologies as part of the diagnosis and treatment; coordinate appointments and procedures; perform direct patient care, nursing documentation, symptom management, patient and family education and counseling Leadership. Form a collaborative practice among the care management team (i.e. surgeons, nurses, social workers, etc.); provide community outreach and education regarding the program; respond to calls concerning the programCollaboration. Work closely with the physician during patient visits; become aware of the treatment plan, expected outcomes and possible treatment complications EDUCATION: BSPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: OT - Occupational Therapist - New York State Education Department (NYSED)PT - Physical Therapist - New York State Education Department (NYSED)RN - Registered Nurse - New York State Education Department (NYSED)NP - Nurse Practitioner Certification - New York State Education Department (NYSED)BLS - Basic Life Support - American Heart Association (AHA)PAY RANGE: $70,000.00 - $100,000.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
